You will receive an email from the Florida Department of Health immediately after leaving your initial evaluation appointment. This email will include important information and instructions for setting up your patient profile and submitting your application. Double check that the information entered is correct as incorrect information can delay the application process. If you opt to send your application via mail, we will provide you with a physical copy and instructions to fill out and send.
