A qualified patient in Florida must meet the 4 requirements listed below:
- Be a Florida resident.
- Have a valid Florida ID.
- Be at least 21 years old.
- Have been diagnosed with a qualifying condition by a previous physician.

- Your medical records from your doctor regarding the condition your seeking treatment for.
- Our patient intake form.
We ask that you please keep these document together and they can be sent us in advance or you can
bring them with you on the day of.

Your Initial Evaluation Appointment
WHAT YOU NEED TO KNOW REGARDING YOUR INITIAL EVALUATION
*Our Evaluations are 100% Risk Free: No Approval, No Charge.
*The physician will review and assess your medical records during this evaluation along with a physical examination.
*If declared qualified by the physician, we will accept payment and you will then be added to the
..Medical Marijuana Use Registry.
*During the same appointment, you will be provided with your patient ID number that will be used to put on your application.
*You will then submit your state application for a medical marijuana card via electronic transmission or mail.
Completing Your Registry Profile & Submitting Your Application
You will receive an email from the Florida Department of Health immediately after leaving your initial evaluation appointment. This email will include important information and instructions for setting up your patient profile and submitting your application. Double check that the information entered is correct as incorrect information can delay the application process. If you opt to send your application via mail, we will provide you with a physical copy and instructions to fill out and send.
Visit A Local Dispensary
You will receive a notice of approval via email, once your application is approved by the state. At this point you will contact and notify our clinic that you have been approved, and our cannabis physician will enter your medicinal marijuana order in the registry. This electronic order sends out to all approved dispensaries in the state of Florida. This leaves you the option to select a dispensary (MMTC) on your own. TO APPLY FOR A MEDICAL MARIJUANA USE REGISTRY IDENTIFICATION CARD, A PATIENT MUST:
1. Be entered into the Medical Marijuana Use Registry by a Qualified Ordering Physician.
2. Complete a paper or electronic Medical Marijuana Use Registry Identification Application.
3. Mail payment or paper application including payment to:

4. Be sure to include your email: Once your Identification Card application has been approved, you will receive a temporary identification via email to be used to fill your order until your physical card arrives
