1. Be entered into the Medical Marijuana Use Registry by a Qualified Ordering Physician.
2. Complete a paper or electronic Medical Marijuana Use Registry Identification Application.
3. Mail payment or paper application including payment to:

4. Be sure to include your email: Once your Identification Card application has been approved, you will receive a temporary identification via email to be used to fill your order until your physical card arrives
